Building Use Form

What follows is an application to rent or use the New Palestine United Methodist Church Facilities.  This form must be filled out when requesting to use any of the church facilities, whether Member or Non- Member. Please fill out the top portion, read the building policy information and initial at the bottom that you have read it and agree.

Once you complete the following form, please: 

1) copy the lower portion of this page, paste it to an email and send to us at newpalumc@comcast.net 

2) mail this form to NPUMC, P.O. Box 377, New Palestine, IN 46163

3) fax it to 317.861.4443 or

4) drop it of in person to the church offices.  The office hours are currently Monday through Friday 9:30 a.m. to 5 p.m.  The Community Center is open later than these hours, until 9 p.m. to be exact, and someone at the Welcome desk would be happy to take your form also.

*If fees are due, an invoice will be mailed to you at the address provided.

·         Any cancellation made within five (5) working days of scheduled event may result in loss of any security deposit.

BUILDING USE POLICY

 

**Please note recent changes to the Building Use Policy.
NO PART OF THE BUILDING OR GROUNDS IS TO BE ALTERED WITHOUT PERMISSION OF THE TRUSTEES.

1.             All facility users are expected to abide by the guidelines.
2.             All facility users, Members/Constituents and Non-Members, are responsible for room setup, take down, and cleanup. Space used should be returned to the state that it was in before setup began. An inventory list shall be provided for each room requested.

3.             All facility users, Members/Constituents and Non-Members, will be required to pay a refundable damage deposit equal to 50% of the total fees due. The deposit will be returned within 10 days after said event if the premises experienced no damages or loss. 
4.             No alcoholic beverages, tobacco products, illegal drugs or course language are permitted in church building or on church grounds.
5.             Children under age 13 must be supervised at all times and remain in the designated scheduled areas while on church grounds or in the building. All parties using the facilities must confine themselves to reserved areas and equipment. Do not move tables or chairs from another area without permission. 
6.             If a scheduling conflict should develop with a church program or ministry, other groups may be asked to move to a different room, seek a different location or cancel the event or meeting.
7.             No food or drink is permitted in the sanctuary with the exceptions of elements for communion and water for speakers/singers. 
8.             No red or purple drinks are permitted in any part of the building except for communion elements.
9.             Church equipment such as TV/VCR, projectors, and sound equipment can be reserved if available. Such needs must be included in the request for reservation form and some equipment may require a trained technician be present at an additional fee.
10.          Any property left on the premises by those using the facility shall, after a period of thirty days from the last usage, be deemed abandoned and shall become the property of NPUMC.
11.          Trash should be placed in the receptacles provided. If trashcans are filled during an event the trash bags should be changed and placed in the dumpster outside.
12.          No signs, banners, flags, streamers, etc., are to be attached or hung from any wall, post or beam within or outside the church without the approval of the trustees.
13.          No paints, tapes, or glues may be used, nor carpentry, electrical or other sorts of construction allowed on the premises without the approval of the trustees.
14.          Do not use supplies belonging to other groups without permission.
15.          Scenery or props must be freestanding. No nails, screws or stage hooks may be used. 
16.          No oil base paint, flammable liquids, fire producing chemicals, and/or open flames in any form is allowed except candles used during worship services. Any use of candles besides those on the altar needs to be approved by the pastor or trustees.
17.          Members scheduling facilities for private, non-ministry events will be charged a use fee.
18.          If your event is changed or canceled please notify the church office immediately or there may be a charge. Any cancellation less than 2 weeks in advance will result in the forfeit of entire deposit, unless extreme conditions are the cause, such as family death, etc.
19.          NPUMC reserves the right to cancel or terminate activities due to emergencies or other unforeseen circumstances.
20.          NPUMC staff members have complete authority during any event held in the church facilities. This includes the right to make disciplinary decisions or dismiss people from the facility in the event that guidelines/policies are being violated.
21.          Use of the facility is at the risk of the participants. NPUMC does not assume liability or responsibility for any member or outside group. NPUMC is not liable for accidents or injuries due to athletic, social, or other events. NPUMC is not responsible for lost or stolen articles. NPUMC does not make any express or implied warranty of the premises, equipment, fixtures or furniture.
22.          Transfer of permission to use the facility to those other than to which this agreement was made is strictly prohibited.
23.          Guideline violations may result in loss of privileges.
